3. Sign the last page of the Policies and Procedures

4. Complete a Medical Care Plan if there is a medical condition

If using Child Care Vouchers, Tax Free Childcare Scheme or The Holiday Activity Fund please enter VOUCHER in the coupon code, and include your reference number in the Voucher Field.

Please note: We require 2 Holiday Activity Fund (HAF) Codes per child, per day.
